Refer a patient
Referring patients to Sora Care is a simple process with big impact.
You’ll benefit from a consultation report to summarize the assessment, treatment plan and follow-up for your patients.
Our expert medical team is dedicated to medical cannabis care and will assess, prescribe, and develop personalized treatment plans for your patients. We’ll provide continued follow-up for as long as your patients are in need.
Please note that we assess and follow patients just for their medical cannabis and cannabinoid treatments.
Your patient remains under your care for all other medical needs.
Assessing a patient for medical cannabis referral
1. Identify a patient candidate
Patients must have tried first-line pharmacological treatment and still have uncontrolled symptoms, side effects or insufficient symptom improvement.
Patients who are self-medicating with medical cannabis and reporting benefits or side effects should receive medical cannabis assessment to confirm safe and effective use.
2. Fill out our referral form and send it our way
We’ll review your referral and contact the patient to book an appointment with one of our Sora Care healthcare practitioners.
There are no fees for patients with a valid healthcare card.
3. Receive a consultation report
We’ll send you a detailed report of the appointment, including your patient’s treatment plan of medical cannabis and prescription cannabinoids, follow-up plan and any other details.
Shared care support
Your medical cannabis patients may require personalized education, registration support or other specific medical cannabis needs.

We’ll contact your patient for a virtual educator appointment within 48 hours and will send you the full report.
